Early Adaptation to the Procedures of Laparoscopic Appendectomy

Abstract
Since early 1990s, Laparoscopic appendectomy(LA) has been suggested as an alternative to conventional appendectomy by many laparoscopic surgeons. Despite the good results published by western surgeons, laparoscopic appendectomy has not gained wide acceptance in the Korea. This may be due in part to the limited data available. After gaining considerable experience with laparoscopic technique through their use in cholecystectomy, we began performing laparoscopic appendectomy for evaluation of early adaptation. We reviewed our early experience of 29 cases of the laparoscopic appendectomy performed during 5 months from August 1994 to December 1994(Group I) and compared its result with those of 23 laparoscopic appendectomy performed during 5 months from January 1995 to May 1995(Group ¥±) at Inha hospital. The demographic finding of two group were no difference. The mean operative time and mean anesthetic time were shorter in Group ¥±. The mean operative time was 51.9 minutes in Group I and 43.5 minutes in Group ¥±, the mean anesthetic time was 75.0 minutes in Group I and 65.2 minutes in Group ¥± respectively(p<0.05). There was no significant difference between two groups in pathologic findings. The mean postoperative hospital stay was 3.4 days in Group I and 3.7 days in Group ¥±. The need of analgesics was less in Group ¥±(Group I; 133.6 mg, 1.7 days Group ¥±; 71.7
mg, 1.1days). 7 cases(24%) in Group I and 9 cases in Group ¥± developed postoperative complications. In conclusion, a surgeon can adapt to the procedure of LA relatively short time, if he had some experience of open appendectomy and other laparoscopic procedures.
